Structural Rhinoplasty

Historically, rhinoplasty surgery was a subtractive process. Surgeons would whittle away bone and remove cartilage to make the nose smaller. Years later, those noses would collapse, leading to pinching and breathing issues. Dr. Brobst utilizes a structural procedure. Instead of simply removing tissue, we reorganize it. We often use your own cartilage, borrowed from the septum or occasionally the rib, to build strong architectural beams inside the nose. This reinforces the structure, ensuring that your aesthetic results stand the test of time.

Open vs. Closed Rhinoplasty

Closed Rhinoplasty: All incisions are made inside the nostrils. This is often reserved for minor changes where structural grafting is not required.
Open Rhinoplasty: A tiny, stair-step incision is made on the columella (the strip of skin between the nostrils). This allows Dr. Bo to lift the skin and view the cartilage and bone directly.
For most patients requiring significant reshaping or airway correction, the open approach provides the visibility needed for precision. The scar on the columella heals exceptionally well, becoming virtually invisible within a few weeks.

The Timeline

Recovery and The Healing Process

Recovery is a commitment. We value transparency, so we want you to know exactly what to expect.

Days 1–3: The Congestion Phase

You will not feel sharp pain; you will feel pressure. Most patients compare it to a severe head cold. You will have a cast on the bridge of your nose and possibly silicone splints inside. You must avoid strenuous activity to keep your blood pressure low and minimize swelling.

Day 7: The Reveal

This is the most exciting day. We remove the cast and sutures in the office. Your nose will be swollen, and you may have some yellowing or bruising under the eyes, but you will immediately see the new shape.

Weeks 2–6: Returning to Routine

By week two, discomfort is minimal. You can return to work and social engagements. While the nose is still healing, it is presentable. You must continue to protect the nose from trauma.

The Long Game: 1 Year

The healing process for rhinoplasty is unique. The skin takes time to "shrink wrap" down to the new underlying framework. While you will see beautiful results at three months, the final definition of the tip requires a full year to settle. Permanent changes take patience.

Insurance typically does not cover cosmetic issues. However, if you have a documented deviated septum or vestibular stenosis causing breathing issues, the functional portion of the surgery (septoplasty) may be covered. We can provide the necessary codes for you to submit to your provider.

Hygiene is critical to prevent infection. We will provide specific instructions on using saline sprays and hydrogen peroxide to keep the incisions and nostrils clean. Do not blow your nose for at least two weeks.

The nasal bones are fragile while setting. If you sustain a significant impact, call our office immediately. We may need to see you to ensure the bone has not shifted.

We can use photos as a communication tool to understand your aesthetic taste. However, copying a nose onto a different face often leads to unnatural results. We aim for a nose that fits your facial features.
